Accommodation Letter Request

  1. Students who are registered with Accessibility Services are to request their Faculty Accommodation Letter at the beginning of each semester. Requests for Faculty Accommodation Letters can be made by completing the Request for Accommodation Letter form or using the button below. Letters will be sent to the student’s Northeast State student email, not D2L email. It is up to the student to either forward their letter to their instructors or they may choose to print them out and hand carry to their instructor.

  2. The Faculty Accommodation Letter is very confidential and should not be discussed with anyone other than the student or Accessibility Services staff.  Faculty should not discuss the student’s disability in front of others. Students should contact Accessibility Services if a faculty member discloses their disability to other students. 

  3. The student is to speak to their instructor individually regarding their accommodations; however, if the student does not communicate with the instructor, the instructor may take the initiative to communicate with the student privately.
